About Procardia (Nifedipine)
Uses and Benefits of Procardia (Nifedipine)
Procardia (nifedipine) is indicated for managing vasospastic angina and chronic stable angina. It helps treat symptoms of vasospastic angina and may reduce angina frequency in patients with chronic stable angina. Procardia can also improve exercise tolerance in those who have symptoms despite using beta blockers or nitrates.
For vasospastic angina, diagnosis includes the following criteria: a classical pattern of angina at rest with ST-segment elevation. Angina or coronary artery spasm triggered by ergonovine and angiographically confirmed coronary artery spasm. Vasospastic angina can be diagnosed based on clinical presentation, even without confirmed vasospasm.
Procardia can be used with beta blockers, but blood pressure should be monitored closely due to the risk of severe hypotension.
Ingredients in Procardia Capsules (Nifedipine)
The active ingredient in Procardia capsules is 10 mg of nifedipine.
Inactive ingredients include glycerin, peppermint oil, polyethylene glycol, soft gelatin capsules (which contain Yellow 6 and may contain Red Ferric Oxide and other inert ingredients), and water. The 10 mg capsules also contain saccharin sodium.
How to Take Procardia (Nifedipine)?
Follow your doctor’s prescription exactly.
Take Procardia capsules as directed. For vasospastic angina, the usual starting dose is 10 mg, three times a day. For chronic stable angina, the starting dose is also 10 mg, three times a day.
Your doctor may adjust your dose based on your needs. The usual effective dose is 10–20 mg, three times daily. Some patients may need 20–30 mg, three to four times a day. The maximum dose is 120 mg per day, though 180 mg/day is rarely needed.
Gradually increase the dose over 7–14 days. Rapid titration may occur based on your condition. Avoid grapefruit and grapefruit juice while using the medication. There is no rebound effect when stopping the medication, but a gradual dose reduction is recommended.
Sublingual nitroglycerin can be used as needed for acute angina. Care should be taken when combining PROCARDIA with beta blockers or nitrates.
Who Should Not Use Procardia (Nifedipine)?
Do not use Procardia if you are allergic to nifedipine or any other ingredients in the capsules. Consult your doctor if you experience signs of an allergic reaction, such as skin irritation, rash, shortness of breath, or swelling of the face or tongue.

Safety Precautions for Procardia (Nifedipine)
Before you take Procardia, tell your healthcare provider if you have low blood pressure, are using beta blockers, have severe coronary artery disease, or have recently had a heart attack. Inform your provider if you have congestive heart failure, aortic stenosis, or are pregnant or breastfeeding.
The medication may cause excessive hypotension, particularly during initial dosage adjustments. Severe hypotension is possible when combined with beta blockers. Avoid using the medication for acute blood pressure reduction. If you’re undergoing coronary artery bypass surgery with high-dose fentanyl anesthesia, allow at least 36 hours for Procardia to wash out before surgery.
In rare cases, Procardia may worsen angina or cause acute myocardial infarction, particularly in patients with severe coronary artery disease. Do not use it within 1-2 weeks after a heart attack or during acute coronary syndrome.
Procardia is not approved for controlling essential hypertension. No controlled studies have defined an appropriate dose for this purpose.
Patients withdrawing from beta blockers may experience increased angina. The medication may worsen this by triggering reflex catecholamine release. Gradually taper beta blockers before starting Procardia.
In rare cases, Procardia may lead to heart failure, especially in patients taking beta blockers. Patients with aortic stenosis may be at higher risk due to reduced benefits from Procardia.
Monitor blood pressure closely, particularly if you take other blood pressure-lowering medications. Mild to moderate peripheral edema, typically in the lower legs, may occur and respond to diuretics. Procardia may mildly reduce platelet aggregation and increase bleeding time. Rare increases in BUN and creatinine have been reported in patients with renal insufficiency.
Avoid grapefruit or grapefruit juice as it may increase Procardia blood concentration. Procardia should only be used during pregnancy if the potential benefits outweigh the risks. Nifedipine transfers into breast milk. Use Procardia during breastfeeding only if the potential benefits justify the risks.
Safety and effectiveness in pediatric patients have not been established. Use in children is not recommended. Elderly patients may have decreased nifedipine clearance, leading to higher drug levels. Dosage adjustments may be necessary due to slower metabolism in older adults.
