About Zofran HCL (ondansetron hcl)

Uses and Benefits of Zofran HCL (ondansetron hcl)

Zofran HCL (ondansetron hcl) is indicated for the prevention of nausea and vomiting caused by cancer chemotherapy and radiotherapy. It helps prevent nausea and vomiting in patients receiving highly emetogenic chemotherapy, including cisplatin doses of 50 mg/m² or more. It is also used for initial and repeat courses of moderately emetogenic chemotherapy.

Zofran HCL is effective in preventing nausea and vomiting in patients undergoing radiotherapy, including total body irradiation, single high-dose abdominal radiation, or daily abdominal radiation.

Ingredients in Zofran HCL (ondansetron hcl)

The active ingredient in Zofran HCL is ondansetron hydrochloride. Inactive ingredients include anhydrous lactose, microcrystalline cellulose, pregelatinized starch (maize), magnesium stearate, triacetin, titanium dioxide, and hypromellose.

How to Take Zofran HCL (ondansetron hcl)?

Follow your doctor’s prescription exactly.

Take Zofran HCL as a tablet, orally disintegrating tablet, or oral solution. The dosage is the same for all forms.

For adults receiving highly emetogenic chemotherapy, take 24 mg once, 30 minutes before treatment. For moderately emetogenic chemotherapy, take 8 mg 30 minutes before treatment, another 8 mg after 8 hours, then 8 mg every 12 hours for 1 to 2 days.

For radiotherapy, take 8 mg 1 to 2 hours before each session. If receiving a single high-dose fraction to the abdomen, take 8 mg before treatment, then 8 mg every 8 hours for 1 to 2 days. For daily fractionated radiotherapy to the abdomen, take 8 mg before each session, then every 8 hours on treatment days.

For postoperative nausea and vomiting, take 16 mg one hour before anesthesia.

For children aged 12 to 17 receiving moderately emetogenic chemotherapy, take 8 mg 30 minutes before treatment, another 8 mg after 8 hours, then 8 mg every 12 hours for 1 to 2 days.

For children aged 4 to 11, take 4 mg 30 minutes before chemotherapy, then 4 mg at 4 and 8 hours after the first dose. After chemotherapy, take 4 mg three times daily for 1 to 2 days.

If you have severe liver disease with a Child-Pugh score of 10 or higher, do not take more than 8 mg per day.

Who Should Not Use Zofran HCL (ondansetron hcl)?

Do not use Zofran HCL if you are allergic to ondansetron or any of its ingredients. Avoid using it if you take apomorphine, as this combination may cause severe low blood pressure and loss of consciousness.

 Safety Precautions

Before you take Zofran HCL, tell your healthcare provider if you:

  • Are allergic to ondansetron or other 5-HT3 receptor antagonists
  • Have congenital long QT syndrome
  • Have low potassium or magnesium levels
  • Have heart conditions like heart failure or a slow heart rate
  • Take medications that prolong the QT interval
  • Use serotonergic drugs, including SSRIs, SNRIs, or MAO inhibitors
  • Are recovering from abdominal surgery or have a risk of bowel obstruction

Tell your healthcare provider about all the medicines you take, including prescription and nonprescription medicines, vitamins, and herbal supplements.

Ondansetron may cause serious allergic reactions, including anaphylaxis and bronchospasm. Stop taking it and seek emergency help if you have trouble breathing, swelling, or severe rash.

Ondansetron can affect heart rhythm, leading to QT prolongation and, in rare cases, Torsade de Pointes. Patients with heart disease or electrolyte imbalances should have ECG monitoring. Do not take ondansetron if you have congenital long QT syndrome.

Serotonin syndrome is a life-threatening condition that may occur when ondansetron is taken with serotonergic drugs. Symptoms include agitation, hallucinations, fast heartbeat, sweating, tremors, and muscle stiffness. Seek emergency help if these occur.

Ondansetron may hide signs of a bowel blockage, especially after surgery or chemotherapy. It does not improve bowel movement and should not replace nasogastric suction. Monitor bowel activity closely.
