About Vantin (Cefpodoxime Proxetil)

Uses and Benefits of Vantin (Cefpodoxime Proxetil)

Vantin is indicated for the treatment of mild to moderate infections caused by susceptible bacteria. It treats acute otitis media caused by Streptococcus pneumoniae (excluding penicillin-resistant strains), Streptococcus pyogenes, Haemophilus influenzae (including beta-lactamase-producing strains), and Moraxella catarrhalis (including beta-lactamase-producing strains). It also treats pharyngitis and tonsillitis caused by Streptococcus pyogenes, but it is not proven for preventing rheumatic fever.

Vantin treats community-acquired pneumonia caused by Streptococcus pneumoniae or Haemophilus influenzae (including beta-lactamase-producing strains). It is also effective for acute bacterial exacerbation of chronic bronchitis caused by S. pneumoniae, H. influenzae (non-beta-lactamase-producing strains only), or M. catarrhalis. Its effectiveness against beta-lactamase-producing H. influenzae in these cases is not established.

This medication treats acute, uncomplicated gonorrhea (urethral, cervical, and anorectal infections in women) caused by Neisseria gonorrhoeae (including penicillinase-producing strains). However, its effectiveness in male rectal infections or pharyngeal gonorrhea is not established.

Vantin is also used for uncomplicated skin and skin structure infections caused by Staphylococcus aureus (including penicillinase-producing strains) and Streptococcus pyogenes. Surgical drainage may be needed for abscesses. The dose for skin infections is higher than for other uses.

It treats acute maxillary sinusitis caused by Haemophilus influenzae (including beta-lactamase-producing strains), Streptococcus pneumoniae, and Moraxella catarrhalis. It also treats uncomplicated urinary tract infections (cystitis) caused by Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Staphylococcus saprophyticus. However, its lower bacterial eradication rate should be weighed against other options.

Before starting treatment, healthcare providers should identify the bacteria and determine their susceptibility to cefpodoxime. Treatment may begin while waiting for test results, but therapy should be adjusted based on lab findings.

To prevent antibiotic resistance, Vantin should only be used for confirmed or strongly suspected bacterial infections. When available, culture and susceptibility results should guide treatment. If not, local resistance patterns may help in selecting therapy.

How to Take Vantin (Cefpodoxime Proxetil)?

Follow your doctor’s prescription exactly.

Take Vantin Tablets with food to improve absorption. Vantin Oral Suspension can be taken with or without food.

For adults and adolescents (12 years and older), the dosage depends on the infection:

  • Pharyngitis/Tonsillitis: 100 mg every 12 hours for 5-10 days.
  • Community-Acquired Pneumonia: 200 mg every 12 hours for 14 days.
  • Chronic Bronchitis Exacerbation: 200 mg every 12 hours for 10 days.
  • Uncomplicated Gonorrhea/Rectal Gonococcal Infections: 200 mg as a single dose.
  • Skin Infections: 400 mg every 12 hours for 7-14 days.
  • Acute Maxillary Sinusitis: 200 mg every 12 hours for 10 days.
  • Uncomplicated Urinary Tract Infections: 100 mg every 12 hours for 7 days.

For infants and children (2 months to 12 years), dosages vary by weight:

  • Acute Otitis Media: 5 mg/kg every 12 hours for 5 days (max 400 mg/day).
  • Pharyngitis/Tonsillitis: 5 mg/kg every 12 hours for 5-10 days (max 200 mg/day).
  • Acute Maxillary Sinusitis: 5 mg/kg every 12 hours for 10 days (max 400 mg/day).

For patients with severe kidney disease (creatinine clearance <30 mL/min), take Vantin every 24 hours. If on hemodialysis, take Vantin 3 times a week after dialysis. No dose adjustment is needed for patients with cirrhosis.

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose.

In case of an overdose, contact a healthcare provider or Poison Control Center immediately.

Who Should Not Take Vantin (Cefpodoxime Proxetil)?

Do not take Vantin if you have an allergy to cefpodoxime or any other cephalosporin antibiotics.

Tell your doctor if you have a history of severe allergic reactions to antibiotics. Signs of an allergic reaction include rash, itching, swelling, dizziness, or trouble breathing. Seek medical help if these symptoms occur.

Side Effects

Common side effects may include:

  • Diarrhea
  • Nausea
  • Abdominal pain
  • Headache
  • Vaginal fungal infections

Serious side effects may include:

  • Clostridioides difficile–associated diarrhea
  • Severe allergic reactions such as rash, swelling, or difficulty breathing
  • Pseudomembranous colitis (severe inflammation of the colon)
  • Severe skin reactions
  • Acute liver injury

This is not a complete list of possible side effects. For a complete list, ask your doctor or pharmacist.

Safety Precautions

Before you take Vantin, tell your healthcare provider if you:

  • have allergies to cefpodoxime, cephalosporins, or penicillins
  • have had a severe allergic reaction to antibiotics
  • have kidney problems

Use caution if you are allergic to penicillin, as cross-allergy is possible. Stop taking Vantin if an allergic reaction occurs. Seek emergency help for severe reactions like swelling, rash, or breathing problems.

Risk of C. difficile-Associated Diarrhea (CDAD)

Vantin, like other antibiotics, may cause CDAD. This condition can range from mild diarrhea to severe colitis. Antibiotics disrupt gut bacteria, allowing C. difficile to overgrow. Symptoms may appear up to 2 months after treatment. If you have diarrhea after antibiotics, contact your doctor. Stop unnecessary antibiotics if CDAD is suspected and seek treatment.

Other Precautions

  • Your doctor may lower your dose if you have kidney disease.
  • Avoid prolonged use, as it may cause new infections.
  • Do not take Vantin without a confirmed bacterial infection. Unnecessary use increases antibiotic resistance.

Important Patient Information

Vantin treats bacterial infections only. It does not work for viruses like colds or flu. Take all doses as prescribed. Do not skip doses or stop early, as this may lead to drug resistance.

Diarrhea is a common side effect. Contact your doctor if you have watery or bloody stools, even up to 2 months after stopping treatment.

Drug Interactions

Tell your healthcare provider if you take:

  • Antacids or H2 blockers, which reduce absorption
  • Anticholinergics, which delay peak drug levels
  • Probenecid, which increases drug levels
  • Nephrotoxic drugs, which may require kidney monitoring

Lab Test Interactions

Vantin may cause a false-positive Coombs’ test.

Use in Specific Populations

  • Pregnancy: Vantin is Pregnancy Category B. Animal studies show no harm, but human data is limited. Use only if clearly needed.
  • Labor and Delivery: Effects during labor are unknown. Use only if necessary.
  • Breastfeeding: Vantin passes into breast milk. Discuss with your doctor whether to stop nursing or the medication.
  • Infants: Safety has not been established in babies under 2 months.
  • Elderly: No dose adjustment is needed if kidney function is normal.

Storage Guidelines

Store Vantin tablets at room temperature between 20°C and 25°C (68°F to 77°F). Keep the bottle tightly closed after each use to protect the tablets from moisture. If using unit dose packs, store them in a dry place away from excessive humidity.
