About Timolol (Timolol Maleate)

Uses and Benefits of Timolol (Timolol Maleate)

Timolol Maleate is indicated for treating hypertension. It can be used alone or with other blood pressure medications, especially thiazide diuretics. It also helps reduce the risk of death and reinfarction in stable patients who have survived the acute phase of a heart attack. Additionally, Timolol Maleate is used to prevent migraine headaches.

Ingredients in Timolol (Timolol Maleate)

The active ingredient in Timolol Maleate is timolol maleate. Inactive ingredients include silicon dioxide, croscarmellose sodium, magnesium stearate, microcrystalline cellulose, pregelatinized corn starch, and sodium lauryl sulfate.

How to Take Timolol (Timolol Maleate)?

Follow your doctor’s prescription exactly.

The usual starting dose of Timolol Maleate is 10 mg twice a day. This can be taken alone or with other medications for high blood pressure, such as a diuretic. Your doctor may adjust the dosage based on your heart rate and blood pressure. The usual maintenance dose is between 20 to 40 mg per day. In some cases, your dose may be increased to a maximum of 60 mg per day, split into two doses. Dosage adjustments should occur at least 7 days apart.

Timolol Maleate can be used with a thiazide diuretic or other blood pressure medications. Your doctor will monitor you closely when starting this combination therapy.

For heart attack prevention, the recommended dosage is 10 mg twice a day after the acute phase.

For migraine prevention, the starting dose is 10 mg twice a day. The dose may be increased up to 30 mg daily, divided into two doses. If needed, the dose can be reduced to 10 mg once daily. If a satisfactory response isn’t achieved after 6-8 weeks of maximum dosage, you may need to stop the medication.

If you miss a dose, take it as soon as you remember, then continue as usual. In case of an overdose, contact your healthcare provider or Poison Control Center immediately, even if there are no symptoms.

Who Should Not Take Timolol (Timolol Maleate)?

Do not take Timolol Maleate if you have bronchial asthma, a history of asthma, or severe chronic obstructive pulmonary disease (COPD). It is also contraindicated if you have sinus bradycardia, second- or third-degree atrioventricular block, overt cardiac failure, cardiogenic shock, or hypersensitivity to this product.

Safety Precautions

Before you take Timolol Maleate, tell your healthcare provider if you: • have liver or kidney problems • have severe renal failure or are on dialysis • have myasthenia gravis or muscle weakness • have cerebrovascular insufficiency or signs of reduced blood flow to the brain • have any allergies, especially to timolol or other medications

Tell your healthcare provider about all the medicines you take, including prescription and nonprescription medicines, vitamins, and herbal supplements.

Precautions for Renal Failure Timolol’s effect on the body is not greatly changed by kidney issues. However, patients with severe kidney impairment on dialysis may experience low blood pressure after a 20 mg dose. Your doctor should be cautious with dosing.

Muscle Weakness Precautions Timolol can worsen muscle weakness, especially in patients with myasthenia gravis or myasthenic symptoms.

Cerebrovascular Insufficiency Precautions Beta-blockers, like timolol, can affect blood pressure and pulse. Use with caution in patients with cerebrovascular insufficiency. If signs of reduced blood flow to the brain appear, your doctor may stop the medication.

Drug Interaction Precautions Timolol can interact with other medications. Always inform your doctor about all medicines you are taking.

Catecholamine-Depleting Drugs If you take catecholamine-depleting drugs like reserpine, Timolol can add to their effects, possibly causing low blood pressure and/or a slow heartbeat, which may lead to dizziness, fainting, or postural hypotension.

Non-Steroidal Anti-Inflammatory Drugs NSAIDs may reduce the blood pressure-lowering effect of Timolol. Your doctor should monitor the effectiveness of the medication.

Calcium Antagonists Oral calcium antagonists can be used with beta-blockers if heart function is normal. Avoid this combination if you have impaired heart function, as it may cause low blood pressure, AV conduction issues, or left ventricular failure. Low blood pressure is more likely with dihydropyridine derivatives, like nifedipine. Left ventricular failure and AV conduction problems are more likely with verapamil or diltiazem. Use intravenous calcium antagonists with caution.

Digitalis and Diltiazem or Verapamil Combining beta-blockers with digitalis and either diltiazem or verapamil can prolong AV conduction time.

Quinidine Quinidine may inhibit Timolol’s metabolism, which can lead to increased beta-blockade effects.

Clonidine If you take both clonidine and Timolol, stop the beta-blocker several days before stopping clonidine. If switching from clonidine to Timolol, wait several days after stopping clonidine before starting the beta-blocker to avoid rebound hypertension.

Risk from Anaphylactic Reaction People with allergies may experience more severe reactions to allergens while taking Timolol. They may also not respond to the usual doses of epinephrine for anaphylaxis.

Carcinogenesis, Mutagenesis, and Fertility Precautions Studies in rats and mice showed increased tumors at very high Timolol doses. The relevance of these findings to humans is unclear. Timolol did not show mutagenic effects in several tests. Reproductive studies in rats showed no issues with fertility.

Pregnancy Precautions Timolol is a Pregnancy Category C drug. Animal studies showed harm to the fetus at high doses, but there are no good studies in pregnant women. Use Timolol during pregnancy only if clearly needed.

Nursing Mothers Precautions Timolol passes into breast milk. Discuss with your doctor whether you should stop breastfeeding or stop the medication.

Pediatric Use Precautions Safety and effectiveness in children have not been established.

Geriatric Use Precautions Studies did not include enough older adults to determine if they respond differently. Start with a low dose in elderly patients, as they may have decreased liver, kidney, or heart function. Monitor kidney function in elderly patients.

Storage Guidelines for Timolol (Timolol Maleate)

Store Timolol at room temperature between 20°C and 25°C (68°F to 77°F). Keep the medication away from light. Dispense the medication in a tight, light-resistant container with a child-resistant closure as defined in the USP. Keep the container tightly closed to maintain its effectiveness.
