Sinemet (Carbidopa/Levodopa) is indicated for the treatment of Parkinson’s disease, post-encephalitic Parkinsonism, and symptomatic Parkinsonism caused by carbon monoxide or manganese exposure. Carbidopa helps patients take lower doses of levodopa by reducing its breakdown outside the brain. Some patients who did not respond well to levodopa alone have improved with Sinemet. This effect is due to carbidopa reducing levodopa breakdown, not a direct effect on the nervous system. Carbidopa does not increase levodopa’s effectiveness but may help reduce nausea and vomiting, allowing for faster dose adjustments.
Ingredients in Sinemet (Carbidopa/Levodopa)
The active ingredients in Sinemet are carbidopa and levodopa. It comes in three strengths: Sinemet 25-100 (25 mg carbidopa, 100 mg levodopa), Sinemet 10-100 (10 mg carbidopa, 100 mg levodopa), and Sinemet 25-250 (25 mg carbidopa, 250 mg levodopa). Inactive ingredients include microcrystalline cellulose, pregelatinized starch, corn starch, and magnesium stearate. Sinemet 10-100 and Sinemet 25-250 also contain FD&C Blue #2. Sinemet 25-100 also contains D&C Yellow #10.
How to Take Sinemet (Carbidopa/Levodopa)?
Follow your doctor’s prescription exactly.
Sinemet (Carbidopa/Levodopa) comes in three strengths: 25-100, 10-100, and 25-250. The usual starting dose is one 25-100 tablet three times a day, providing 75 mg of carbidopa daily. Your doctor may increase the dose by one tablet daily or every other day until you take up to eight tablets per day. If using the 10-100 strength, the starting dose is one tablet three or four times a day. Your doctor may increase the dose until you take two tablets four times a day.
Switching from Levodopa Stop taking levodopa at least 12 hours before starting Sinemet. The starting Sinemet dose should be about 25% of your previous levodopa dose. If you took less than 1,500 mg of levodopa daily, you may start with one 25-100 tablet three or four times a day. If you took more than 1,500 mg daily, you may start with one 25-250 tablet three or four times a day.
Maintenance Dose Your doctor will adjust your dose to get the best response. You should receive at least 70-100 mg of carbidopa daily. If you need more carbidopa, your doctor may switch you to 25-100 tablets. If you need more levodopa, your doctor may switch you to 25-250 tablets. The 25-250 dose may increase by one-half or one tablet daily or every other day, with a maximum of eight tablets per day.
Monitoring Your doctor will monitor you closely when adjusting your dose. Sinemet works faster than levodopa alone, which may cause involuntary movements sooner. If this happens, your doctor may lower your dose. Eyelid spasms (blepharospasm) may be an early sign of taking too much Sinemet.
Other Parkinson’s Medications You can take Sinemet with other Parkinson’s medications, except levodopa without a decarboxylase inhibitor. Your doctor may adjust your dose if needed.
Stopping Sinemet Do not stop or lower your Sinemet dose without medical advice. Stopping suddenly can cause serious problems, including fever and confusion. Your doctor will monitor you closely, especially if you are taking neuroleptics.
Surgery You can continue taking it before surgery as long as you can take fluids and medicine by mouth. If you stop taking the medication temporarily, your doctor will monitor you for symptoms similar to neuroleptic malignant syndrome (NMS). Resume your usual dose as soon as you can take oral medication.
Who Should Not Use Sinemet (Carbidopa/Levodopa)?
Do not use Sinemet (Carbidopa/Levodopa) if you take nonselective monoamine oxidase (MAO) inhibitors. Stop these medications at least 2 weeks before starting Sinemet. You may take Sinemet with an MAO-B inhibitor, such as selegiline, at the recommended dose.
Do not use Sinemet if you are allergic to any of its ingredients. Do not take Sinemet if you have narrow-angle glaucoma.

Risk of Fever and Confusion Stopping or lowering Sinemet too quickly can cause a serious reaction like neuroleptic malignant syndrome (NMS). Symptoms include fever, stiff muscles, confusion, sweating, and a fast heartbeat. Seek medical help if you notice these symptoms.
Other Precautions Your doctor should monitor your liver, kidneys, heart, and blood function. If you have glaucoma, regular eye pressure checks are needed.
Sinemet may cause involuntary movements, hallucinations, or unusual behaviors. Tell your doctor if you notice changes in your thoughts or actions. Some patients develop strong urges, like gambling, spending, or increased sexual urges—report these to your doctor.
People with Parkinson’s disease have a higher risk of melanoma. Get regular skin exams.
