About Sensipar (Cinacalcet)
Uses and Benefits of Sensipar (Cinacalcet)
Sensipar (Cinacalcet) is indicated for treating secondary hyperparathyroidism (HPT) in adults with chronic kidney disease (CKD) on dialysis. It lowers parathyroid hormone (PTH) levels to help manage the condition. Sensipar is not for CKD patients who are not on dialysis due to a higher risk of low calcium.
Sensipar also treats high calcium levels in adults with parathyroid carcinoma. It helps lower calcium levels in the blood. Additionally, Sensipar treats high calcium in adults with primary HPT who need surgery but cannot undergo a parathyroidectomy.
Ingredients in Sensipar (Cinacalcet)
The active ingredient in Sensipar is cinacalcet hydrochloride, which helps lower parathyroid hormone (PTH) levels and calcium in the blood.
Inactive ingredients include pre-gelatinized starch, microcrystalline cellulose, povidone, crospovidone, colloidal silicon dioxide, and magnesium stearate. The tablets have a green film coating (Opadry® II green), a clear film coat (Opadry® clear), and a layer of carnauba wax.
How to Take Sensipar (Cinacalcet)?
Follow your doctor’s prescription exactly.
Take Sensipar with food, shortly after a meal. Swallow the tablet whole—do not chew, crush, or split it.
For secondary hyperparathyroidism in dialysis patients, the usual starting dose is 30 mg once daily. Your doctor will check calcium and phosphorus within 1 week and iPTH within 1 to 4 weeks after starting or adjusting the dose. Your doctor may increase the dose every 2 to 4 weeks to 30, 60, 90, 120, or 180 mg once daily. The goal is an iPTH level of 150 to 300 pg/mL. Take iPTH tests at least 12 hours after Sensipar. The tablet can be used alone or with vitamin D or phosphate binders. Your doctor will monitor calcium levels and may adjust your medications or stop them if calcium is too low.
For parathyroid carcinoma and primary hyperparathyroidism, the usual starting dose is 30 mg twice daily. Your doctor may increase the dose every 2 to 4 weeks to 30 mg twice daily, 60 mg twice daily, 90 mg twice daily, or 90 mg up to 4 times daily. The goal is to maintain normal calcium levels. Calcium levels should be checked within 1 week of starting or adjusting the dose.
For patients switching from Parsabiv to Sensipar, stop Parsabiv for at least 4 weeks before starting Sensipar. Calcium levels should be normal before starting Sensipar. The starting dose is 30 mg once daily.
Your doctor will monitor calcium levels regularly. Dialysis patients usually have calcium checked monthly, while other patients are checked every 2 months. If calcium falls below 7.5 mg/dL but remains above this level, your doctor may prescribe calcium or vitamin D. If calcium drops below 7.5 mg/dL or symptoms appear, your doctor may stop the medication. Once calcium is 8.0 mg/dL or higher and symptoms resolve, It may be restarted at a lower dose.
Who Should Not Take Sensipar (Cinacalcet)?
Do not take Sensipar if your blood calcium is below the normal range. Your doctor will check your calcium levels before starting treatment.

Safety Precautions for Sensipar (Cinacalcet)
Before you take Sensipar, tell your healthcare provider if you have low calcium levels (hypocalcemia) or symptoms such as tingling, muscle spasms, or seizures. Have a history of heart problems, including congenital long QT syndrome or a family history of sudden death. If you have a seizure disorder, the medication may lower the seizure threshold. Have a history of stomach or intestinal bleeding, ulcers, or severe vomiting. Are you taking other medicines that lower calcium levels.
The medication can cause low calcium levels, which may lead to life-threatening complications. Symptoms include tingling, muscle pain, muscle spasms, seizures, or heart problems. This medication is not approved for children or for chronic kidney disease (CKD) patients who are not on dialysis.
Low calcium can cause QT prolongation, which may lead to life-threatening heart rhythm problems. Patients with certain heart conditions, such as congenital long QT syndrome or a family history of sudden death, are at higher risk. Your doctor will monitor your calcium levels and heart rhythm.
Taking the tablet with other calcium-lowering drugs can cause severe hypocalcemia. Your doctor will monitor your calcium levels closely. If calcium is too low, you may require calcium supplements, phosphate binders, or vitamin D. Your physician may adjust or discontinue it if needed.
Sensipar can cause upper gastrointestinal bleeding, especially in patients with risk factors such as ulcers, gastritis, or severe vomiting. Your doctor will monitor for worsening nausea, vomiting, or signs of bleeding.
Patients with heart conditions may experience low blood pressure, worsening heart failure, or irregular heartbeats. These effects may be related to low calcium levels.
Adynamic bone disease can occur if parathyroid hormone (iPTH) levels drop too low. If iPTH falls below 150 pg/mL, your doctor may lower your Sensipar or vitamin D dose.
Tell your healthcare provider about all the medicines you take, including prescription and nonprescription medicines, vitamins, and herbal supplements.
