Rytary (carbidopa/levodopa) is indicated for the treatment of Parkinson’s disease. It also treats post-encephalitic Parkinsonism. Rytary treats Parkinsonism that may follow carbon monoxide or manganese intoxication.
Ingredients in Rytary (Carbidopa/Levodopa ER)
The active ingredients in Rytary extended-release capsules are carbidopa and levodopa. Each capsule contains one of the following combinations: 23.75 mg carbidopa and 95 mg levodopa, 36.25 mg carbidopa and 145 mg levodopa, 48.75 mg carbidopa and 195 mg levodopa, or 61.25 mg carbidopa and 245 mg levodopa.
Inactive ingredients include microcrystalline cellulose, mannitol, tartaric acid, ethyl cellulose, hypromellose, sodium starch glycolate, sodium lauryl sulfate, povidone, talc, methacrylic acid copolymers, triethyl citrate, croscarmellose sodium, and magnesium stearate. The capsule shells contain gelatin and titanium dioxide. Blue capsule components contain FD&C Blue #2 and yellow iron oxide. Yellow capsule components contain yellow iron oxide. Capsules with black imprinting contain iron oxide black, while those with blue imprinting contain FD&C Blue #2.
How to Take Rytary (Carbidopa/Levodopa ER)?
Follow your doctor’s prescription exactly.
If you are new to Levodopa, start with 23.75 mg/95 mg three times daily for the first 3 days. On day 4, increase to 36.25 mg/145 mg three times daily. Your doctor may adjust your dose. The maximum dose is 97.5 mg/390 mg three times daily. Some patients may need up to 5 daily doses based on their response. The goal is the lowest effective dose to reduce side effects like nausea and dyskinesia. The maximum daily dose is 612.5 mg/2450 mg.
Switching from Immediate-Release Carbidopa-Levodopa
Doses of this extended-release capsule are not the same as other carbidopa/levodopa doses. Your doctor will calculate your total daily levodopa dose and use it to determine your starting dose. Any strength of the extended-release capsule may be used after switching. Your doctor will adjust the dose and dosing frequency as needed. Keep other Parkinson’s medications the same during this transition. This medication was given 3 to 5 times daily in studies. If you take carbidopa/levodopa with a COMT inhibitor, you may need a higher dose of this medication. The extended-release capsule has not been studied with other levodopa products.
Stopping the Medication
Do not stop this medication suddenly or reduce the dose too quickly. Your doctor will gradually lower your dose when stopping treatment.
How to Take the Capsules
Swallow the capsule whole with or without food. High-fat meals may delay levodopa absorption by about 2 hours. Do not chew, split, or crush the capsules. If you have trouble swallowing, open the capsule and sprinkle the contents onto 1 to 2 tablespoons of applesauce. Eat the mixture immediately and do not save it for later.
Who Should Not Use Rytary (Carbidopa/Levodopa ER)?
Do not use Rytary if you take a nonselective monoamine oxidase (MAO) inhibitor, such as phenelzine or tranylcypromine. Do not use Rytary if you have taken a nonselective MAO inhibitor within the past 2 weeks. Taking these drugs together can cause high blood pressure. Consult your doctor for more information.
